If you need to make a claim you will need to fill in a claim form. You can get one by:

• Calling 0800 300 889
• Downloading a form now.
• Writing to us at: MORE TH>N, Freepost SCE 8009, Ceefa Deefa House, 35 Friday St, Henley-on-Thames, Oxon, RG9 1ZW.

The form is simple to complete and it’s clear who should complete each section - you or your vet.

Once completed, return the form to the FREEPOST address above. We usually deal with all pet insurance claims on the day we receive them.

Any claim will be settled by cheque.
